Juvenile Justice Release Planner: Bilingual Position-DDAP

CJCJ is seeking a dedicated and energetic Juvenile Justice Release Planner to join our Youth Justice Services team, to serve and support youth and transitional-aged young adults. They will work to reduce unnecessary institutionalization and promote individualized interventions.

The Release Planner is responsible for tracking the San Francisco Juvenile Hall population daily, connecting with youth in detention, researching and developing individualized alternative release plans for youth currently in secure detention, presenting and securing alternative releases and coordinating to facilitate a successful and smooth reentry. The Release Planner will partner with community-based providers to link the youth to services immediately upon reentry. Linkages will be based on the young person’s unique needs, strengths, and interests. Their role post reentry is to provide ongoing support to the youth and to ensure the plan is fully implemented and successful, until the youth is thriving with their newly developed support network. The Release Planner will work in partnership with community members to ensure a comprehensive approach to success. They will also work with various justice stakeholders to include defense attorneys, judges, probation officers, and prosecutors. They will regularly be working in the juvenile hall, in the office, and in the field, in San Francisco and the surrounding local area.

Qualifications include

• Strong interpersonal communication skills and engages well with young adults
• Bilingual Spanish-speaker preferred
• Experience in juvenile/​criminal justice, case management, social work, substance abuse, non-profit programming, or other related fields
• Education in criminal justice, social work, law, public policy, psychology, sociology, creative writing, journalism, English literature, or related field (Master’s degree preferred)
• Excellent writing, research, and organizational skills with the ability to work under pressure
• Excels when working with people from diverse backgrounds with experience working with justice-impacted individuals
• Detail-oriented with great time management
• High energy and strong initiative; ability to work independently as well as in a team
• Experience developing case plans preferred and knowledge of community-based service providers is a plus
• Passion for justice with a good sense of humor
• Maintain a valid driver’s license and access to a registered and insured vehicle to use in carrying out your job duties

Duties & Responsibilities Include

• Assess detained youth to identify their unique reentry needs and individualized goals
• Research, develop, and expeditiously write pre-release and post-entry plans, and present such plans to justice stakeholders, i.e. courts, defense attorneys, probation officers
• Provide ongoing direct reentry support as well as linkages for necessary services including physical/​mental healthcare, employment, housing, basic needs, life-skills development, mentorship, and social/​recreational services
• Coordinate and effectively communicate with the client, their family, and key professionals (e.g., case managers, social workers, attorneys, educators, and service providers) to ensure a successful reentry and long-term support
• Serve as the client’s key advocate; attend Child and Family Team meetings, court hearings, and I.E.P. meetings with schools; evaluate their progress in meeting goals; and make adjustments to further support their ongoing success
• Research and build on current knowledge of available community resources and utilize a unified system of care through a network of community-based organizations
• Maintain case files and excellent documentation while tracking client progress and outcomes
• Prepare regular reports containing outcomes and varying data to track program progress
• Develop and submit monthly reports and attend court hearings
• Carry out all responsibilities in a professional and timely manner
